Kailyn is excited about her marriage to Javi and the upcoming birth of her second child. However, as she starts to build a life with her new family, the conflicts with her son Isaac's father Joe are far from over, and they continue to battle over custody and visitation rights. Kailyn hopes to work out their differences for Isaac's sake, but Joe is making it more difficult than she anticipated. This season is full of unexpected surprises as Kailyn's estranged mother reaches out hoping to reconnect and work on their relationship before she gives birth to her son Lincoln. Kailyn is apprehensive and struggles with letting her mother back into her life.

Chelsea is eager to begin a new chapter in her life as she starts beauty school, but the moment is bitter sweet as she faces the fact that Aubree is growing up and about to begin pre-school. Despite trying to focus on school, Chelsea is once again involved with drama involving her ex Adam who is starting a new family with his girlfriend. Although Chelsea tries to take the high road she can't help but feel resentment towards Adam since he's always in and out of Aubree's life. Things take a dramatic turn when Adam files for visitation rights and wants Aubree be a part of his new family.

Leah has her hands full with twins Ali and Aleeah and new baby Addie. Stress levels are heightened when Ali is diagnosed with a rare form of muscular dystrophy, which will likely get worse as she gets older and cause her to rely on a wheelchair. Leah and Corey must remain strong for their girls and to give Ali a head start, they decide to enroll her in pre-school early. Meanwhile, Aleeah has been acting out since her sisters are getting more attention than usual. Leah and Jeremy hit a bump in their relationship as Jeremy's long stints away from home start to take a toll. With their marriage on the rocks, Leah suggests they go to counseling.

After a short stint in rehab, Jenelle's struggle with addiction continues to impact her decisions and every day is a battle. Her soon to be ex-husband is in jail and she has to make the most difficult decision of her life. Even though she is finding it hard to leave the past behind her, she is trying to make a clean break to stay sober for Jace. Jenelle and Barbara still do not see eye to eye on Jenelle's choices but she is spending more time with her family and is happier than she has ever been. While trying to stay on track, Jenelle meets ex-Marine Nathan who changes her life for the better. Jenelle and Nathan quickly fall in love, move in together and begin their life as a new couplewith a big surprise. However, even as their relationship becomes more serious Nathan and Jenelle struggle with jealousy and money problems.

The teen birth rate in the U.S. declined 29 percent between 2007 and 2012. Despite the steep decline in teen birth rates, more than 700,000 teenage girls in the U.S. still become pregnant each year and nearly one out of every ten children in the U.S. is born to a teenage mother. The vast majority of these pregnancies are unintended, and one quarter of all teen moms have a second child within two years. In a 2012 poll by the National Campaign to Prevent Teen and Unplanned Pregnancy, 77 percent of teens age 12-19 who have seen "16 and Pregnant" and "Teen Mom" say the shows "help teens better understand the challenges of pregnancy and parenting." More information about sexual health issues affecting young people, including unintended pregnancy, is available atwww.itsyoursexlife.com, a public information resource developed by MTV and the Kaiser Family Foundation. The National Campaign to Prevent Teen and Unplanned Pregnancy is working with the show to provide additional information.
