According to the B.C. Coroners service, Canadian authorities have revealed that late actor Cory Monteith passed away from "a lethal combination of heroin and alcohol."

The report notes that "Post-mortem testing, which included an autopsy and toxicological analysis, found that Mr. Monteith, aged 31, died of a mixed drug toxicity, involving heroin and alcohol. It should be noted that at this point there is no evidence to suggest Mr. Monteith's death was anything other than a most-tragic accident."

The actor was found dead on Saturday, July 13th in his Vancourver hotel room.

Monteith, who was dating his Glee co-star and Broadway vet Lea Michele, starred in the film Monte Carlo, opposite Leighton Meester, Selena Gomez and Katie Cassidy. His other film credits include FINAL DESTINATION 3, THE INVISIBLE, DECK THE HALLS, and WHISPER. He left rehab in April of 2013, after voluntarily admitting himself to a treatment facility for substance addiction in March of 2013.

His television credits include a lead role on the series "Kaya" and a recurring guest-star role on KYLE XY. He has also made numerous guest appearances on television shows, including SMALLVILLE, SUPERNATURAL, STARGATE, FLASH GORDON, and INTERNS. Additionally, he starred in the 2007 television movie HYBRID.

Born in Calgary, Alberta, Canada, Monteith moved to Vancouver Island at a very young age. Throughout his life he has held jobs as a Wal-Mart greeter, a school bus driver, a roofer and a cab driver, before pursuing a career in acting.
